Registration

The registration form is available as a printable pdf file.

Participants can receive six (6) contact hours toward Continuing Education Units (CEU) or Continuing Professional Development Units (CPDU).

Registration fee of $75.00 must accompany the form. All forms must be received by October 18th. Make checks payable to Constitutional Rights Foundation Chicago. Please note that if you register for the workshop and for any reason are unable to attend, your registration fee cannot be refunded.
